This form is for Film Societies only. It is not possible to include a Film Festival in the same application for membership. Stand-alone NPV Film Festivals must use a different form.

To satisfy our obligations to film and DVD distributors this application must be completed annually. (The FVFS Membership year is 1st January to 31st December)

Your affiliation fee as indicated, is for a film society only, for one calendar year, and is based on the peak membership in the previous full calendar year. (For new societies it is based on your anticipated membership.)
If you run several groups or streams, add their memberships, but if a person is in two groups don't count them twice.

This is where you screen to two or more groups at different times with different memberships. It might include a "Golden Oldies" or "Tuesday matinees" stream for example. It may not be the same program! You will have to pay rights for each screening.

CAREFUL, as this may affect what you pay for DVD rights! We will assume this to be 60% of your membership stated above, unless you wish to advise otherwise. If you have several groups, please let us know the average for each group. It you screen each DVD several times tell us the average total watching each DVD.

IMPORTANT NOTE. If your society wishes to make use of the discounted DVD screening rights charges through the ACOFS DVD rights agreement, you must screen to members only, with no charges made for individual screenings. (ie they must be non-theatrical screenings).

Note that for 16mm or DVD screenings any visitors should not be charged for entry, and their entry should be limited to once per year. For DCP screenings a small number of visitors by paid admission is acceptable because he screening is considered "commercial". See also note to Q 14 B.

Note that the ACOFS agreement does not apply to DCP screenings, and is not necessary for DVD screenings to friends in private homes, and is not applicable when anyone is charged for admission to the screening.

Unless screening to friends in a private home, you must have permission to screen a DVD. If you are screening using DCP technology or charge for admission to screenings, you should be paying commercial rates.
